Very few have run off grooves (in Ireland at least). I would say that when these do occur they could be natural erosion caused by overflow.

Hob made one a while back with modern tools but smooth by hand with a stone, I believe. Very lovely it is too!

The conical ones are all limited to Carlow (where the grooved standing stones are too, but I don't think there's a connection) and can reach 2 feet deep.
